Pharmacists acting as an extension of physicians’ care teams is a key component of the tech-enabled, integrated care team I described above. Medication errors, poor adherence, incomplete treatment plans, and lack of communication around prescriptions have significant implications on the health of patients and contribute to rising costs. By integrating the PCP with the pharmacist and creating a closely connected care experience with the patient, we can create extra bandwidth; we can elevate satisfaction; we can improve care delivery and outcomes. All stakeholders win. This is a huge part of the work we’re doing at Catalyst Health Group because we understand and have seen the immense value that comes from this extended, integrated care model.
